North Carolina Formerly Incarcerated Transitions Services

A Direct Support Professional works directly with the formerly incarcerated individual after release to achieve successful reentry and specifically to facilitate connections with wraparound community resource providers, and if needed behavioral health and substance use treatment through services that include the following:

  • obtaining vital records
  • obtaining identification
  • securing employment
  • housing placement
  • transportation services
  • social services benefits

The overall goal of Formerly Incarcerated Transitions Services is to help the formerly incarcerated evaluate their own lives, make decisions about how they want to live, and help them implement those goals.

Formerly Incarcerated Transitions Services require North Carolina Medicaid coverage or Blue Cross Blue Shield GIFT Insurance.

Assertive Community Treatment

An individual who is appropriate for CDH's Assertive Community Treatment does not benefit from receiving services across multiple, disconnected providers and may become at greater risk of hospitalization, homelessness, substance use, victimization and incarceration.

Services in the program are determined on an individual basis, but may include:

A Direct Support Professional assists an individual in advancing toward personal goals with a focus on enhancing community integration and regaining valued roles (example, worker, daughter, resident, spouse, tenant, or friend).

Because individuals may be court-ordered to receive Assertive Community Treatment services, a CDH direct support professional will thoughtfully carry out planned assertive engagement techniques including rapport-building strategies, facilitating meeting basic needs and motivational interviewing techniques. These techniques are used to identify and focus on the individual’s life goals and what he or she is motivated to change.

It is the Direct Support Professional's responsibility to monitor the individual’s mental status and provide needed supports in a manner consistent with the individual’s level of need and functioning. The direct support professionals deliver all services according to a lived-experience recovery-based philosophy of care. The team promotes self-determination, respects the person receiving care as an individual in his or her own right and engages peers in promoting hope that the individual can recover from mental illness and regain meaningful roles and relationships in the community.

Commercial insurance and North Carolina Medicaid is accepted through Cedric Dean Holdings, Inc, as well as self-pay options including sliding fees.
